The Spartan Community Health Center is located in the northwest wing of Lima Senior High School and is open to all district students, staff and their families.
The center is a partnership between the Lima City Schools and Health Partners of Western Ohio (HPWO).
Primary care and dental services are available at the center. There is also a behavioral health provider to complete depression, anxiety, alcohol use and other appropriate screenings for patients.
Immunizations, work and sports physicals are also available. Patients also have access to HPWO's full retail pharmacy.
Students seen at the center either need a signed consent from their parent or guardian prior to receiving treatment, or a two-person parent/guardian verbal consent over the phone at the time of service.
Click on the link to the left to get to the consent form to allow your child to be treated at the center.
The center is open from 8 a.m. to 4:30 p.m. Monday through Friday when school is in session. Reach the center at 419-996-3436.
